If you are going abroad, we ask that you complete a holiday form to assess whether you need any vaccinations. The form is available at our reception desk. If vaccinations are required, an appointment will be made with the practice nurse to administer them, along with any information available on the areas in which you are travelling. NB – there will be a charge for some vaccines. We also sell Travel Packs to any patients travelling abroad. Please ask the practice nurse for details.
